ⓘ READING THE CONSTELLATION
A cluster = one system or data source. Its color is the system; its size is visual weight, not a record count.
Lines inside a cluster tie its parts together. Lines between clusters are data flows named in the meeting.
Almost every flow lands in Arcadia Foundry at the center.

Click any node and the right rail opens that system: what it holds, who uses it, and what the room said about it.SHOW A GAP

| Layer | Platform | Used for |
|---|---|---|
| Network / CIN enablement | Arcadia Vista, Authoring, Conversational AI | Population insights, risk and contract performance, payer reporting, VBC quality, cross-FQHC benchmarking, network capacity |
| Care management workflow | Arcadia CareWeb | Assessments, risk stratification, automated tasks, care plans, care transitions (Medicaid SCM + TCM; Medicare CCM / TCM) |
| Health center | Azara DRVS · local EHR | Patient-level insights, UDS / quality reporting, care gaps and registries, referral and prior auth, operational and financial impact |
| Data aggregation | Arcadia Foundry, AWS, VedaPointe, Foresight | Claims, beneficiaries, eligibility and assignment, ADT / HIE, EHR data |
What the room said about the feeds
- Only Arcadia receives EHR / ADT and LabCorp data; other vendors get payer-file copies or forwards.
- NC HealthConnex sends ADTs only, no CCDs. Medicare LabCorp feed not live yet.
- Azara is live at ~15 health centers; a new center goes live in ~2 weeks.
- ECW FHIR is underway, but scheduling (SIU) needs a separate, costly adapter.
- CMHN's own AWS holds raw and historical data back to 2012.
- Neither Arcadia nor Azara queries the national network for CCDs.
Frequency is read from how people described their work in the meeting, not from login data.
"Partial" means a screen touches it (often with sample data) but the workflow the room described isn't built.

UDS results come from the recording. HEDIS rows, trends and pins are sample data. Click a row to drill to centers, providers and patients.
Change is vs the prior trailing year; for the A1c > 9 measure a fall is an improvement. Download arrows export the patient list for that measure.
Gap = denominator − numerator. To target = patients still needed to reach the target. Green = primary target met, amber = secondary, red = not met.
Rows are rendering providers and rooms exactly as the EHR schedule names them. Counts are real; patients, gaps and no-show scores in expanded rows are examples.

Program management · are the members receiving the care they should?
Beneficiaries reached is shaded by value. Switch the denominator at the top to read reach against enrolled members instead of everyone on the roster.
Care manager productivity · who is delivering the care?

Keep eligible patients covered after January 1, 2027
New North Carolina Medicaid requirements start Jan 1, 2027. Community health centers need to find patients at risk of losing coverage, decide what each one needs, help them through the requirements and work with county DSS to resolve cases.

NCCHCA aims
What the pilot combines
Why NCCHCA: it represents every FQHC in North Carolina and already works with NC Medicaid, the managed care plans and local, state and federal partners, so the model can scale statewide.
